What is the difference between an HMO, PPO, and EPO?
These are types of health insurance plans with different rules about which providers you can see: • HMO (Health Maintenance Organization): Requires a primary care physician and referrals for specialists. Generally only covers in-network care. • PPO (Preferred Provider Organization): More flexibility — you can see any doctor, in or out of network, without a referral. In-network costs are lower. • EPO (Exclusive Provider Organization): Like a PPO but only covers in-network care; no out-of-network coverage except emergencies.
