My health records show information that is out of date. Why?

AskMD displays what is documented in your connected health system. If a condition has been resolved or a medication has been discontinued but your provider hasn't updated their records yet, the old information may still appear. This is a documentation issue on the provider side — contact your provider's office to request an update.
